Most failed automation projects fail for the same handful of reasons, and none of them are "the AI wasn't good enough." They are decisions made before the build even started.

Automating a broken process

Automation makes whatever process it touches faster. If that process was already broken — unclear ownership, inconsistent steps, no real definition of "done" — automation just makes the broken version run faster and harder to catch.

Skipping the approval question

Every automation involving AI needs a clear answer to "what happens when it's unsure, and who gets asked before something goes to a customer." Projects that skip this end up with an automation that occasionally sends something wrong, with nobody positioned to catch it in time.

Starting with the flashiest project instead of the highest-friction one

Businesses often want to automate the most impressive-sounding workflow first. The better first project is usually the most repetitive, most annoying, lowest-risk one — it proves the approach works and builds internal trust before anything customer-facing is on the line.

No owner after launch

An automation with nobody assigned to watch it, adjust it, and fix it when the business process around it changes will quietly degrade. Software does not maintain itself just because it is labeled "automated."

Treating it as a one-time project instead of an ongoing system

The business changes, the workflow changes, and the automation needs to change with it. Budgeting only for the build and never for the maintenance is how a good automation becomes a liability eighteen months later.
